1. surat tawaran
2. salinan surat beranak
3. salinan ic sendiri
4. salinan ic ibu/bapa
5. salinan slip spm
6. 5 gambar saiz passport
7. salinan slip gaji
8. pakai baju pinafore (last year form 5 one)

* pendaftaran mula jam 8.oo am
*tiada fesyen rambut dibenarkan
*the clerk said a prefect will show you the way to go for registration

*This form 6 letter will not be sent to your house this year.. so you need to check your name in form 6 list online at http://apps.emoe.gov.my/form6/semakanform6/ starting from May 6.

Don't go to private institution for Form 6
Because it will be hard for you to enter local university. In fact a lot people said like that including the Lowyat.net forum.
